Summary of Conversation

How to structure a conversation with the patient at the same time you are reviewing their technique. The purpose of this conversation is to identify potential intentional non-adherence, explore understanding of COPD (perceived need) and value (perceived benefit) of medications.

Summarize their perceived need and benefit of inhalers, then seek permission to offer information on what they can do to help them stay at home longer (reduce risk of coming back to hospital with a COPD flare-up).

Use TeachBack method to confirm that patient/family understands. Ask if they have any concerns or can think of anything that may make it difficult for them to use. Ask the patient how confident (x/10) are they that they will use their inhaler? Lastly, as you are speaking with the patient, assess if they have the capacity to identify yellow level symptoms of a COPD action plan (or Asthma action plan).

1. Identifying potential non-adherence of long acting agents for COPD

2. Explore patient’s understanding / beliefs of COPD; What is their perceived need?

3. Explore patient’s understanding of how this inhaler helps them? What is their perceived benefit?

4. Seek permission to offer them information that can help them

5. Explain how regular use will help the patient

6. Explore patient’s understanding of why proper technique is important

7. Use TeachBack to confirm the patient understood.

8. Does the patient have any concerns? How confident are they of using their inhaler?

9. Before coming to hospital, did the patient take any additional medications for their worsening COPD symptoms or adjust their doses of their steroid inhaler for worsening asthma symptoms?

10. Future ability to use an action plan? Does the patient/family have capacity to identify worsening of COPD/asthma symptoms so they could follow an action plan?
